    The Association Between Risk Factors And Ultrasound-Based Grades Of Non-Alcoholic Fatty Liver Disease In Type-2 Diabetes Patients

    Background: Non-alcoholic fatty liver disease (NAFLD) has become more common as the cause of cirrhosis and liver cancer. The liver disease is highly prevalent in people with type-2 diabetes. Indonesia is not spared from the global epidemic of type-2 diabetes. The ultrasound examination is clinically easy-to-use, economical and non-invasive as a tool to detect NAFLD, compared to the gold standard, liver biopsy. To date, there has been no study in Indonesia to link risk factors and ultrasound-based severity grading of NAFLD. Aim: To understand the association between risk factors and ultrasound-based grades of NAFLD in patients with type-2 diabetes. Method: The present study was an observational study with a cross-sectional design (May-October 2018) that involved 82 type-2 diabetes outpatients of the internal medicine clinic in the Gotong Royong Hospital (Surabaya, Indonesia). The risk factors assessed were gender, age, diabetes duration, obesity (anthropometric measurement: body mass index/ BMI, waist circumference and waist-to-hip ratio), glycemic control (hemoglobin A1c/ HbA1c level) and dyslipidemia (lipid profile: total cholesterol, low-density lipoprotein/ LDL, high-density lipoprotein/ HDL and triglyceride). The ultrasound-based grades of NAFLD consisted of grade 0 (no NAFLD), grade 1 (increased liver echogenicity with normal images of intrahepatic vessel lines and diaphragm), grade 2 (blurred image of intrahepatic vessel lines) and grade 3 (blurred images of intrahepatic vessel lines and diaphragm). Statistical p-value was significant at ≤ 0.05. Results: Seventy-eight subjects (95,1%) had NAFLD. The ultrasound-based NAFLD grades were significantly different across age groups (Kruskal-Wallis) but the Spearman’s rank correlation test result was not significant. Body mass index and total cholesterol were positively correlated (r = 0.390 and 0.237, respectively) with the NAFLD grades. Conclusion: Higher BMI and total cholesterol are associated with increased ultrasound-based NAFLD grades

    Menganalisis pengaruh pemberian ekstrak etanol sambiloto (Andrographis paniculata) terhadap persentase perlemakan hati, kadar serum glutamic oxaloacetic transaminase (SGOT)/ serum glutamic pyruvic transaminase (SGPT) dan profil lipid tikus Wistar jantan yang dipapar diet tinggi lemak. Metode : Rancangan penelitian adalah the randomised post test only control group design. Total besar sampel adalah 50 ekor tikus putih Rattus norvegicus strain Wistar jantan yang terbagi secara acak dalam 5 kelompok (randomisasi). Uji normalitas yang digunakan adalah uji Kolmogorov Smirnov ( = 0,05). Uji homogenitas yang digunakan adalah uji Levene ( = 0,05). Uji beda dengan menggunakan uji Anova (analysis of variance) ( = 0,05) atau uji Brown- Forsythe ( = 0,05). Uji korelasi yang digunakan adalah uji Pearson ( = 0,05). Hasil : Pemberian ekstrak etanol sambiloto dengan dosis 100, 200 dan 400 miligram/ kilogram berat badan dapat menurunkan persentase perlemakan hati (r = -0,950), kadar SGOT (r = -0,964)/ SGPT (r = -0,917) dan kadar kolesterol total (r = -0,923)/ TG (trigliserida) (r = -0,973)/ LDL (low-density lipoprotein) (r = - 0,960) serta meningkatkan kadar HDL (high-density lipoprotein) (r = 0,956) tikus putih yang dipapar diet tinggi lemak Simpulan : Peningkatan dosis ekstrak etanol sambiloto dapat menurunkan persentase perlemakan hati, kadar SGOT/ SGPT dan kadar kolesterol total/ TG/ LDL serta meningkatkan kadar HDL tikus putih yang dipapar diet tinggi lema

    The aim of this study was to analyze the effects of ethanol extract of sambiloto (Andrographis paniculata) on fatty liver percentage, serum glutamic oxaloacetic transaminase (SGOT) level and lipid profile of wistar strain white rat exposed to high fat diet. The study used randomized post test only control group design. Total sample was 50 male wistar strain rats (Rattus norvegicus) divided randomly into 5 groups (randomization). The normality test used was Kolmogorov Smirnov test (a=0.05). The homogeneity test used was Levene test (a=0.05). The comparative test was done using Anova test (analysis of variance) (a=0.05) or Brown-Forsythe test (a=0.05). The correlation test was done using Pearson test (a=0.05). The administration of sambiloto ethanol extract with doses of 100, 200 and 400 milligrams (mg)/kilogram (kg) body weight (BW) decreased the percentage of fatty liver (r=-0.950), SGOT (r=-0.964)/SGPT (r=(R=-0.973)/LDL (low-density lipoprotein) (r=-0.960) and increased HDL (high-density lipoprotein) levels (r=-0.923)=0.956) in white rats exposed to a high-fat diet. In conclusion, increased dose of ethanol extract of sambiloto can decrease the percentage of fatty liver, SGOT/SGPT and total cholesterol/TG/LDL and increase HDL level of white rats exposed to high fat diet

    Background and aims: Meals with high protein and fiber could reduce weight and improve diabetes risk factors. Isomalto-oligosaccharide (IMO), a form of dietary fiber, could induce the afferent signal that causes appetite suppression. However, the direct effect of fiber supplementation in the form of IMO combined with a highprotein diet (HPF) on those parameters is still unknown. This study aims to investigate the effect of HPF on anthropometric parameters and blood glucose regulation of healthy subjects. Methods: Thirteen healthy subjects were given a hypocaloric high protein diet (HPD) mixed with their prepared meals for two weeks. Followed by the HPF diet for another two weeks. Their anthropometric parameters, such as body composition (total body weight, body fat percentage, and fat-free mass), BMI and waist circumference, and fasting plasma glucose, were measured. Results: Compared to pre-intervention, HPF could significantly (p ≤ 0.004) reduce the anthropometric parameters and fasting plasma glucose. Compared to HPD, HPF could significantly (p ≤ 0.005) reduce more total body weight, body fat percentage, and BMI. In addition, HPF could induce more satiety than HPD (higher VAS score). Conclusion: HPF could improve the subject’s anthropometric parameters which is obviously beneficial in preventing the risk of developing diabete
